1. What is Home Health Care?
Home Health Care
is one-on-one care for you, the patient, in the comfort of
your own home. Home Health Care provides all the services listed under
"SERVICES" ("Home Health", "Hospice", and "Private Care").
This
service provides you with the care necessary so that you may not be
confined to a hospital room or nursing facility.
A nurse
will come to your home and do an "assessment" on you and then report back
to your doctor. The doctor will then determine what treatment will be
needed and the nurse and doctor will work together to see that all of your
medical needs are met. The nurse over your case will have daily
communication with your doctor if necessary.

2. Who Qualifies for Home Health Care?

3. What Is The Process?
After your doctor
orders home care, you will be given a thorough assessment of your home
health needs by a qualified member of our team. This evaluation can take
place in the hospital, nursing home, or in your home.
Next, a home health
team member will get together with your doctor to put together a treatment
plan tailored to your specific needs.
Finally, your home
health case manager will discuss the treatment plan with you and your
family, and care is ready to begin.
Our agency will stay in continual contact
and communication with your doctor, with you, and with your
family.
Our services are available 24 hours a
day, 7 days a week, including all holidays.
We welcome you or your family members to
call us at any time to discuss any concerns or answer any questions. We
are always available to you.

Me?
Yes!
5. Who Chooses the Home Health Agency I'm To
Use?
YOU! Your doctor or hospital case manager
or nursing home can provide you with a list of agencies in your area. You
have the right to choose.

8. Who Governs Home Health Agencies?
All Home Health Agencies are
licensed by the state to provide care within the minimum health and safety
standards established by regulation and rule. The Department of
Aging and Disability Services (DADS) enforces the
standards by periodically conducting unannounced surveys of these
agencies.
